LGD-4033 is an investigational, orally bioavailable, nonsteroidal selective androgen receptor modulator (SARM) characterized by high-affinity binding to the ligand-binding domain of the androgen receptor (AR) with preferential anabolic activity in skeletal muscle and bone tissue. Following receptor activation, it is being investigated for its ability to promote androgen receptor conformational signaling, nuclear translocation, and interaction with androgen response elements (AREs), leading to transcriptional modulation of genes associated with myofibrillar protein synthesis, satellite cell activation, nitrogen retention, and maintenance of lean tissue mass.

Its tissue-selective pharmacology is being studied for the potential to enhance anabolic signaling in musculoskeletal tissue while minimizing activity in non-target tissues relative to traditional anabolic agents. Preclinical and early clinical investigations have explored its role in supporting increases in lean body mass, improvements in functional strength parameters, recovery capacity, and preservation of bone mineral density through osteoanabolic mechanisms.

Additional research is examining its pharmacokinetic advantages, including oral bioavailability, sustained receptor occupancy, and favorable dosing convenience, as well as its broader applications in conditions involving muscle wasting, catabolic stress, sarcopenia, and impaired musculoskeletal performance. How it works

LGD-4033 exerts its investigational pharmacological activity via high-affinity agonism of the androgen receptor (AR). Following ligand–receptor binding, the AR undergoes conformational activation, dissociation from cytosolic heat-shock proteins, dimerization, and nuclear translocation. The activated receptor complex subsequently binds androgen response elements (AREs) within genomic DNA, modulating transcription of genes involved in myogenesis, ribosomal biogenesis, myofibrillar protein accretion, satellite cell activation, and osteoanabolic signaling. Its tissue-selective profile is hypothesized to arise from differential co-regulator recruitment, receptor isoform expression, and localized transcriptional responses within skeletal muscle and osseous tissue.
